Documentation Requirements

On your first visit to Werkman, Boven & Associates, we ask you to complete our Patient Information Form, including the Patient’s Consent. You can download the Patient Information Form here and bring it along to your first appointment. You also can complete and submit this form online by way of an email invitation. For the email invitation, contact our clinic with your email address.

Visit Length

The length of each footcare appointment varies based on several factors, which will be determined at the time of booking.

Cancellation, No-Show and Lateness Policy
  • To provide efficient services to all patients, our office requires 24-hour notice for cancelling or rescheduling appointments. This ensures that we can provide quality, timely services to all our patients.
  • A $10 fee will be charged for appointments cancelled with less than 24 hours’ notice. A $20 fee will be charged for missed appointments. All fees for cancellations or missed appointments are the patient’s responsibility.
  • Late-comers may be asked to reschedule their appointment for another day, and appropriate charges for the late or missed appointments may apply.
